LANL incident probed

SANTA FE – A U.S. Department of Energy office is investigating what’s described as a “near miss to a fatality” at Los Alamos National Laboratory, apparently after a worker went into a room despite the sounding of a low-oxygen alarm. https://www.abqjournal.com/1105482/los-alamos-lab-incident-probed.html

An employee was almost killed and no one has any comments about this?
Must be so common under LANS that it is not worth comment.
